By signing this waiver, I acknowledge and agree to the following:

Assumption of Risk: I understand that swimming and pool-related activities involve inherent risks, including but not limited to slipping, falling, drowning, and other injuries. I assume all risks associated with my participation or the participation of my child(ren)/guest(s).

Release of Liability: I release and hold harmless Bluegrass Swim Club, its staff, board, volunteers, and members from any and all liability for injury, loss, or damage resulting from my use or my guest’s use of the pool facilities.

Supervision: I agree to supervise all minors in my care and ensure they follow all pool rules. Lifeguards will be present at all times.

Health & Conduct: I confirm that I and any guest(s) are in good health and will act responsibly while on club property.
